If you are a new patient, please come twenty minutes prior to appointment for paperwork. Existing patients do have to fill out a new health history every year. Online forms coming soon!
Financial Policy
It is important that you are able to fit the dental care you need into your financial life, and we offer a variety of payment options to help you do just that, including:
Cash and personal checks
Visa, MasterCard, Discover, and American Express
No-interest, monthly payments through CareCredit
Additionally, we are happy to file your dental benefits for you; however, remember that your dental benefits are an agreement between you and your employer. Any remaining balance or unpaid fees are your responsibility. Any estimated patient fees or deductibles are due at the time of service.
Care Credit
Our office accepts CareCredit, a healthcare credit card that is used to pay for out-of-pocket expenses not covered by dental insurance. CareCredit is subject to credit approval, but once you are accepted, you can use it again and again. CareCredit is a great option if you're on a budget because you can pay for your dental expenses in installments. Ask us about CareCredit today!
